What this app is

Design Your Wellness is a wellness tracking companion for patients participating in Life Design Wellness programs. It helps patients track meals, nutrition estimates, weight, sleep, activity, blood pressure, medications, and progress.

The app is not the clinic's electronic medical record. Clinical records and official chart information remain in Elation or other clinic-approved systems.

Information handled by the app

The app handles account details, wellness tracker entries, meal notes, selected food photos, security and reliability signals, and support requests you choose to send.

Food photos may be processed by the app's server and configured estimation providers to generate nutrition estimates. Do not upload unrelated medical documents or sensitive photos.

How information is used

Information is used to operate the app, save tracker data, estimate nutrition, show progress, provide support, protect accounts, and connect patients to booking and patient portal resources.

The app does not sell personal information. The app does not provide medical advice, diagnosis, treatment, emergency monitoring, or medical-device functionality.

Retention and deletion

Patients may request deletion of the mobile app account and companion tracker data using the account deletion page. Some records may be retained for legal, security, backup, audit, or clinic operational reasons.

Elation chart records are separate medical records managed by the clinic and are not silently deleted by deleting the mobile app account.
