App support

For help with the Design Your Wellness mobile app, use the clinic contact form and mention the mobile app. Do not include urgent medical concerns or highly sensitive details in a general support message.

Clinic questions

For appointment, membership, or clinic communication questions, use the clinic contact page or the patient portal. For medical emergencies, call 911 or seek emergency care.

Patient portal

The app links to Elation Passport for patient portal access. The portal is separate from this companion app and is the right place for official clinic communication when enabled by the clinic.
