How to request deletion

Use the clinic contact form and write that you want to delete your Design Your Wellness mobile app account. Include the email address used for the app account.

Do not send passwords, payment details, medical record documents, or emergency medical concerns through the contact form.

What can be deleted

A deletion request can remove or deactivate the mobile app account and companion tracker data, including patient-entered meal, weight, sleep, activity, blood pressure, medication, and goal entries.

What may be retained

Some information may be retained for security, legal compliance, audit logs, backups, dispute resolution, or clinic operational requirements. Backup copies may take additional time to expire.

Official medical records in Elation are separate from this companion app. Requests about medical-record amendment, access, or retention should be handled through the clinic's medical-record process.

Expected timing

Deletion requests are reviewed after the clinic verifies the request. Timing can vary based on identity verification, legal retention requirements, and backup schedules.
